- Applicants notified of acceptance – March 22nd, 2011
- An email will be sent to your address listed on the primary application indicating your admissions status into the program. Confirmation instructions and form will be included.
- Confirmation form and fee ($100) DUE – April 8th, 2011
- Secondary Application DUE – May 2nd, 2011
- Complete secondary application AFTER you have been accepted and have submitted your confirmation form and fee.

- Health Information Form (online)
- 1 passport size photo (mail to address below)
- Copy of unofficial transcript (print from UAccess and mail to address below)
- Copy of passport (mail to address below)
- Reference: Your reference will be contacted using the information provided on the General Application. He or she will receive instructions on how to complete the online reference form, which is due as part of the primary application. It is the applicant’s responsibility to follow-up with his or her reference to ensure timely completion of the online form.
